CONSENT FORM AND AGREEMENT

I, hereby grant permission to Atra N America Humanity Corporation to use my image, likeness, and/or voice in photographs, videos, and other media for the purpose of promoting Atra N America Humanity Corporation on social media platforms, websites, and other promotional materials.

I understand that my participation is voluntary and that I will not receive any compensation for the use of my image, likeness, and/or voice. I also understand that Atra N America Humanity Corporation may edit, alter, copy, exhibit, publish, or distribute these materials for purposes of publicizing Atra N America Humanity Corporation's programs or for any other lawful purpose.

I hereby waive any right that I may have to inspect or approve the finished product or products and the advertising copy or other matter that may be used in connection with them or the use to which they may be applied.

I release, discharge, and agree to hold harmless Atra N America Humanity Corporation and its agents from any and all claims, demands, or causes of action that I may have by reason of this authorization or release.

I am 18 years of age or older and have read and understood this release before signing it.
